The Role
We’re seeking a mission-driven Clinical Case Manager, LCSW – Older Adults to join our Complex Care team, a collaborative, multidisciplinary group dedicated to improving outcomes for older adults with complex medical, behavioral health, and psychosocial needs.
In this role, you’ll support a panel of patients using a whole-person, patient-centered approach—blending clinical case management with behavioral health interventions. You’ll partner closely with primary care providers, nurses, behavioral health clinicians, and community partners to help patients navigate a complex healthcare system and overcome barriers to wellness.
You’ll report to the Director of Complex Care and be a trusted voice in shaping how we deliver thoughtful, effective care to older adults.

Has experience with (or strong familiarity with) housing resources and navigation in Marin County (a big plus!).
What You’ll Do
Manage a caseload of 30–40 Complex Care patients.
Develop comprehensive, collaborative care plans with patients.
Coordinate medical, behavioral health, and social services.
Provide targeted behavioral health support and interventions.
Address social determinants of health (SDOH) and other barriers to care.
Connect patients to community resources and supports.
Conduct home and community visits as needed.
Triage and respond to care needs in a timely, thoughtful manner.
Collaborate with an interdisciplinary care team (primary care, nursing, behavioral health, care navigation, and external partners).
Participate in case conferences, trainings, and administrative meetings.
Document and report data accurately and on time.
Support satellite clinics and work occasional extended clinic hours when needed.
Share insights to improve case management services and quality of care.
Travel between clinic sites or community locations as required.

What You Bring
Required
Licensed Clinical Social Worker (LCSW) with an active California license.
Fluency in English (written and verbal).
Computer literacy, including Microsoft Office and internet research.
Ability to write clear, professional documentation.
Valid California Driver’s License.
Preferred
1–2 years of experience working with individuals with complex medical, behavioral health, and/or substance use needs.
Experience working with older adult populations.
Knowledge of legal, ethical, and regulatory standards related to patient rights and confidentiality.
