SUMMARY OF POSITION:
The Discharge Registered Nurse (RN) is responsible for coordinating and implementing discharge planning activities to ensure timely, safe, and effective patient transitions from the hospital or clinical setting to home or other care environments. This role collaborates with multidisciplinary teams, patients, and families to prepare discharge instructions, patient education, and post-discharge care plans. The Discharge RN ensures regulatory compliance, quality outcomes, and continuity of care through detailed communication and documentation.
MINIMUM QUALIFICATION(S):
Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
Minimum of one (1) year of clinical nursing experience; discharge planning or case management experience preferred.
AHA BLS prior to end of orientation period
