Discharge Planner II - Behavioral Medicine
DescriptionSummary:The Discharge Planner is responsible for negotiating, procuring, and coordinating appropriate services and resources needed by patients after the acute inpatient phase. This role is capable of performing non-complex discharge planning tasks, such as returning patients to nursing homes, ordering durable medical equipment (DME), coordinating Home Health services, and faxing referrals to placements.Responsibilities:Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.Assists with appropriate interventions, working collaboratively with all healthcare team members regarding the need for discharge planning and available options
