Job DescriptionGeneral SummaryThe Home Infusion Navigator manages the care and service of patients being discharged to a home infusion setting. He/ she works collaboratively with physicians and other members of the health care team to achieve the highest quality clinical outcomes with the most efficient use of available resources. The Home Infusion Navigator assumes responsibility for interfacing directly with clinicians within referral sources to facilitate a home infusion referral.
