The Community Care (CC) Coordinator Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. The nurse demonstrates proficient practice and decision-making, deliber planning, and critical thinking skills. Basic Requirements: English Language Proficiency.
