Job DescriptionGeneral SummaryThe Home Infusion Navigator manages the care and service of patients being discharged to a home infusion setting. He/ she works collaboratively with physicians and other members of the health care team to achieve the highest quality clinical outcomes with the most efficient use of available resources.
