This Social Worker is assigned to the HBPC Program of VA Central Iowa Healthcare System with assigned duty location at the Community Based Outpatient Clinic in Mason City, Iowa. The incumbent is responsible for assessment, planning, and provision of case management services and has responsibility to provide direct services for all adult and geriatric Veterans enrolled in this program.
