The New Mexico VA Healthcare Network is recruiting for a Senior Social Worker assigned to the Homeless Patient Aligned Care Team (HPACT)/Health Care for Homeless Veterans (HCHV) program . The incumbent will manage homeless program consults, conducts assessments, and develops follow-up plans for Veterans experiencing homelessness.
