Nurse Navigator - Performance Improvement
SUMMARY: Under the direction of the Director of Quality and Care Continuum, works in a collaborative fashion with patients/families/caregivers, multidisciplinary team members, as well as postacute providers to ensure every patient receives comprehensive, timely, safe, cost-effective, and quality healthcare services. Proactively works autonomously and in team settings along with excellent communication skills with patients/families/caregivers and providers. EXPERIENCE: Minimum of two years of orthopedic nursing experience. Working knowledge of alternative payment methodology, insurance coverage, postacute providers, and community resources.
