The incumbent serves as a program coordinator for Dementia Care and Traumatic Brain Injury (TBI) providing outpatient case management and care coordination at the Salem Veterans Affairs (VA) Health Care System (HCS). This specialty treatment program is designed to provide preventative and proactive continuous patient-centered TBI and dementia care interventions that are evidence-based. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
