PCMHI-Collaborative Care Manager provides care for Veterans enrolled at all North Florida/South Georgia NF/SG sites. PCMHI-CoCM work collaboratively with Mental Health providers and Primary Care Patient Aligned Care Teams (PACT) to provide collaborative/coordinated stepped Mental Health care. These plans are focused on health promotion and mental health disease management utilizing evidence based process re-design to produce the best possible outcomes. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
