The Patient Access Coordinator is responsible for effective coordination, monitoring and facilitation of services necessary to meet the acutely hospitalized Veterans' medical and/or health needs in the community. The Patient Access Coordinator assists in care coordination for VA services, referrals and consults based on Veterans' eligibility status and services offered at VA Puget Sound Health Care System (VAPSHCS). The coordinator must also manage and facilitate transfers into VAPSHCS. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
