The RN Case Manager will increase the use of provider time to focus on patient management. Improve access to GMH psychiatrists and nurse practitioners by providing daily management of complex psychosocial needs. Conducts reviews of the patient's medical record and other documents, ensuring that there is open communication between providers engaged in the care of Veterans. The RN GMH CM will work closely with a variety of healthcare professionals, clinical services, internal and external services, community agencies to provide efficient and effective health care using the appropriate resource management criteria. He/she acts on behalf of the Veteran to ensure that necessary clinical services are received, and that progress is made.In addition, this RN possesses the knowledge and skills to: Communicate and interact appropriately and courteously with all internal and external customers Maintain confidentiality of electronic, written, and/or verbal patient/employee information.
