PCMHI-Collaborative Care Manager provides care for Veterans enrolled at all North Florida/South Georgia NF/SG sites. PCMHI-CoCM work collaboratively with Mental Health providers and Primary Care Patient Aligned Care Teams (PACT) to provide collaborative/coordinated stepped Mental Health care. These plans are focused on health promotion and mental health disease management utilizing evidence based process re-design to produce the best possible outcomes. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: 3 years Mental Health experience preferred 1 year care/case management preferred Ability to manage high volume of patients Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Incumbent must possess the mental and physical ability to satisfactorily perform the duties of the proposed assignment. Physical requirements include visual acuity, keen hearing, clear distinctive speech, and manual dexterity. May involve potentially long periods of lifting, carrying, pulling, pushing, reaching above shoulder, walking, standing, sitting, kneeling, and repeated bending. Transferring patients and objects may be required. Safe patient handling equipment is provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive, sit and/or ride in GSA-vehicles for long periods of time. In-direct patient care work is mostly sedentary. Typically the employee sits comfortably to complete the work. However, there may be some walking, standing, bending and carrying of light items such as patient records and supplies. In-direct care staff may be assigned to work in direct care areas as needed. ["Duties include but not limited to: Employment contingent on completion of pre-work for training (Phase 1) and VHA PCMHI COMPETENCY TRAINING (Phase 2) within first year of employment and passing Phase 3 Testing (Three months post training) and (Six Months post training) with no more than 3 attempts each. Successfully managing a panel of 90-120 Veterans per month within 4 months of start date to PCMHI Collaborative Care Manager position. Performs 30-40 assessments per week. Incumbent must be able to make sound clinical decisions, practicing at the high end of their license. Facilitate treatment plan changes for patients who are not improving as expected whom might need more intensive or more specialized mental health care, in consultation with the medical provider and the psychiatric consultant. Facilitate referrals for clinically indicated services (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Develop and complete relapse prevention self-management plan with patients who have achieved treatment goals and are soon to be discharged from care management. Preferred care/case management experience. Experience with screening for common mental health and/or substance abuse disorder. Ability to work with patients via phone, video and also in person. Must have critical thinking skills to manage a difficult Veteran population. Working knowledge of evidence based psychological treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem solving treatment, behavioral action). Basic knowledge of pharmacology for common mental health disorders that is within appropriate scope of practice for RN. Experience with evidence-based techniques. Administrative Duties: Maintains patient confidentiality and privacy in accordance with HIPAA and PCMHI policy. Serves as a patient advocate and educate patients and families as clinically indicated. Possesses knowledge of hospital and PCMHI policies. Demonstrates ability to collaborate and communicate effectively in a team setting. Ability to maintain effective and professional relationships with patient and other members of the care team. Assists in gathering data for QI/PI processes. Promotes PCMHI care management to all sites in NF/SG. Occasional travel, you may be expected to drive to all clinic sites within NF/SG. Performs other tasks as assigned.
