The role you’ll contribute: The value you’ll bring to the team:
The agency has a need for weekend coverage that frequently includes a combination of skills to cover essential functions on the weekend that are usually covered by other staff during the week. This may include intake of new referrals; functioning as an "admission" nurse/performance of essential visits for care that must be provided on the weekend, on-call coverage to address unanticipated issues and reduce preventable rehospitalizations, scheduling duties, and serve as an expert resource for other home care staff. Depending on the size of the agency, the weekend nurse may perform some or all of the above functions. The Home Health RN – Weekend is a professional nurse able to function independently, exercising good judgment related to provision of home care services on the weekend. Functions under the general supervision of the agency Manager. May also perform as the on-call nurse providing RN oversight and 24-hour availability for the agency. May coordinate staff working the weekend including other RN's, therapists, Licensed Practical Nurses and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with nursing standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. Coordinates and directs the home care patient's care based on individual patient needs. Workdays and functions will be mutually determined between employee and agency Director; and will exceed minimum requirements for exempt status professional employee.
JOB DUTIES
• May perform intake function for incoming agency referrals, entering data into the computer, verifying physician orders,assessing payer and patient for appropriateness.
• Functions as an admission nurse, completing admission process for new patients as well as performs home nursing visits as assigned
.• Provides comprehensive assessment, planning, implementation and evaluation of patients assigned
.• Reports changes in patient condition in a timely manner to the physician and/or RN Care Manager or supervisor.
.• Provides a handoff report to the patient's assigned home care case manager who will be assuming care of the patient
.• Follows orders for treatment and provides services as indicated on the plan of care.
• Utilizes and implements appropriate educational tools and pathways per guidelines and/or policy and
QualificationsThe expertise and experiences you’ll need to succeed:
KNOWLEDGE AND EXPERIENCE PREFERRED:
• A working knowledge of community resources and an ability to refer patients and families appropriately• Home Care Regulations and Third-Party Reimbursement as it impacts care delivery• Current IV Therapy skills
EDUCATION AND EXPERIENCE REQUIRED:
Associate and 1+ years of relevant clinical RN experience
EDUCATION AND EXPERIENCE PREFERRED:
• Recent, relevant experience in a Medicare-certified home health agency as a visit nurse
• Bachelor's degree in nursing
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Cardio Pulmonary Resuscitation
Driver's License
