The Cincinnati, OH VA Medical Center is recruiting for a full time Senior Social Worker (Home Based Primary Care). This position is assigned to Social Work service. The incumbent in this position will serve as a core member of the interdisciplinary Home-Based Primary Care (HBPC) treatment team which is a specialized program under the administration of Primary Care.
