The Patient Flow Coordinator is responsible for ensuring the patients admitted to the VA Palo Alto Health Care System (VAPAHCS) meet the requirements for the appropriate level of care for the specific treatment plan regardless of location or services, collaborates with Utilization Management as a benchmark, ensuring patient is meeting criteria for admission and or continued stay. Is clinically and administratively responsible to the Nurse Manager of the Clinical Command Center. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Applies a collaborative team approach in identifying, analyzing and resolving patient flow issues. Promoting continuity of care through collaboration with patients, families and healthcare teams. Participates in interdisciplinary team rounds/huddles and discharge planning to ensure timely and efficient patient movement with anticipated discharges. Participates in education activities to improve clinical knowledge and skill and maintains expertise in management of patient flow. Assesses patient flow needs capacity, availability, acuity skill level of personnel working in collaboration with Nurse Managers in assigning responsibilities to meet needs that may impede patient flow. Demonstrates positive, effective communication skills and professional behavior to promote cooperation and teamwork to enhance patient flow. Effectively communicates information to appropriate staff in a timely manner regarding patient flow issues. Exhibits leadership and collaborates with the interdisciplinary team to improve and sustain the quality and effectiveness of services to patients in an interdisciplinary setting. Participates in designing, implementing, monitoring and revising systems to ensure the cost effective use of resources including staffing effectiveness and enhancing patient care utilizing System Redesign principles. Works collaboratively with ACHO Management Team to identify and promote staff development and enhance staff competencies, and System Redesign initiatives. Collaborates with the Assistant Chief and the Chief Nurse of ACHO for Program Development, Quality Care, Performance Measures and Resource Planning. Applies Lean principles and attends Dyad Tier Checking meetings as scheduled 70% of the time. Seeks to maintain open communication, productive working relationships and teamwork with all persons involved with the care and flow of patients.
