The position works for the James A. Haley Veterans' Hospital and Clinics (JAHVH) in Tampa, Florida, as a direct report to Social Work Service, and is assigned to the Physical Medicine and Rehabilitation Program. This independent position provides care coordination, discharge planning, psychosocial & family support services to Veterans, Service Members & their Family Members/Caregivers.
