As the RN Case Manager, you'll support a diverse member population with education, advocacy and connections to the resources they need to continue their care while reducing risk for readmissions. 

Primary Responsibilities:

• Support the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction 
• Partner with social workers to coordinate care and discharge planning 
• Manage a designated case load to:
  • Assess patient care and treatment plans 
  • Promote efficient utilization of resources
  • Remove barriers and intervene as necessary 
  • Provide patient education
  • Coordinate with various referral sources to ensure proper continued care post-discharge

This position is located at SSM Health, Good Samaritan Hospital in Mount Vernon, IL. The Case Manager will work typical day hours with rotating weekends and holidays based on staffing.

Required Qualifications:

• Active and unrestricted RN license in the state of Illinois 
• 3+ years of hospital-based acute care experience 
• Ability to work rotating weekends and holiday schedule based on staffing 
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation

Preferred Qualifications:

• BSN Degree 
• Certified Case Manager (CCM)
• Experience working as a case manager  
• Proven ability to maintain a case load/workload of 25-35 patients 
• Proven ability to work in an evolving environment with competing priorities with the ability of being adaptable to change
