Job Summary - CORRECTIONAL HEALTH The Case Mgr Discharge Plan-CORHLT facilitates the collaborative interdisciplinary process of case management encompassing assessment, planning, facilitation, care coordination, and evaluation appropriate to the scope of licensure.
