RN Quality PI Coordinator FT Days Position Summary:
Professional nurse who, as a result of education and in-depth clinical experience possesses the advanced clinical knowledge and skills necessary to provide nursing leadership, medical staffing and case management and with targeted populations. Provides coordination/utilization of economic, technological and human resources towards meeting regulatory requirements for Core Measures population. Communicates at the inter departmental level as well as across all health care areas with the goal of improving clinical management of Core Measures patients thereby impacting medical and psychosocial care across the health care continuum
Provides nursing leadership, medical staffing and case management and with targeted populations.
Provides coordination/utilization of economic, technological and human resources towards meeting regulatory requirements for Core Measures population.
Communicates core measures at the interdepartmental level as well as across all health care areas.
Improves clinical management of core measures for patients thereby impacting medical and psychosocial care across the health care continuum.
Performs comprehensive assessment using regulatory guidelines and other appropriate criteria for designated levels of care that may include acute inpatient and observation.
Coordinates with all staff necessary to develop a mechanism to monitor patient progress against guidelines and evaluates variances for individual patients and targeted populations.
THE RN QUALITY PI COORDINATOR FT DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.
Required: 5 years recent case management or acute care, and/or community nursing experience preferably in with core measure/data collection experience.
