The role you’ll contribute:
The Case Manager is responsible for the timely facilitation of quality healthcare services to meet patient specific needs, promotes positive patient outcomes, effectively use resources and contain healthcare costs.
The value you’ll bring to the team:
Review inpatient & observation admissions and continues stays for appropriateness concurrently and retrospectively
Identify opportunities for process improvement
Communicate with patient, family, physician and interdisciplinary team regarding discharge planning
Identify at risk patients for discharge needs early in admission, initiate contact with patient/family and proactively refer for necessary resources
Educate staff regarding Case Management's role in the interdisciplinary approach to patient care
The expertise and experiences you’ll need to succeed:
Current GA RN license
One to two years minimum nursing care preferred in hospital case management
