As a Case Manager with us, you will have the opportunity to be involved in the coordination of care and services for patients admitted to Platte Valley Medical Center / SCL Health who are identified as needing assistance or meeting Care Management criteria.

Your day.

As a Social Work Case Manager you need to know how to:

• Coordinate the care and services for patients identified as needing assistance or meeting care management criteria.


• Assess the patientâ™s prior level of functioning, access to and/or use of community resources and available support systems.


• Identify appropriate admission and continued stay issues.


• Attend, facilitate, and participate in rounds and case conferences.


• Identify needs, facilitate, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles.


• Participate in multidisciplinary groups and development of guidelines.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

• Graduate of an accredited school of social work, sociology, or related field


• Minimum of one (1) year of clinical experience

Preferred:

• At least two (2) years of case management experience in a hospital setting
