ResponsibilitiesThe Medical Care Coordination (MCC) program is seeking to hire a Case worker. Primary duties include:
•Address patient’s socioeconomic needs •Assist the Medical Care Manager (MCM) and the Patient Case Manager (PCM) with patient monitoring and updating care plan results •Follow-up with service linkages •Monitor and follow-up with patients to track outcomes •Serve as the liaison between HIV Counseling, Testing sites and the medical clinic to ensure that new patients are enrolled in medical care seamlessly and in a timely fashion
Qualifications•Bachelor’s in Social Work, Counseling, Psychology, Marriage and Family Counseling, Human Services •Experience in HIV/AIDS specific social services •Demonstrated ability to conduct a manualized intervention program •Demonstrated ability to develop a network of health and social support service providers that serve PLWHA •Demonstrated ability to act as a liaison between patients, specialists, and community partners
Not required, but a bonus if you have the following: •Bilingual in English and Spanish
