You Will Be Responsible For:
Serves as liaison between AdventHealth CFD and University partnerships as the primary point of contact
Coordinates all aspects of University classes offered at various University settings including instruction, curriculum planning, use of University electronic course platforms, grading, and oversight of all students
Leads, develops, coaches, and mentors assigned University students in the field setting
Frequently reviews and trends available data and reports in order to performance manage students in a timely and supportive manner
Maintain a positive work environment that influences efficiency and attainment of service metrics and key performance indicator goals.
Manages program and University students in a manner that meets applicable regulatory licensure and accrediting regulations.
Responsible for implementation of new processes within the department specific to the University health coaching program, evaluates current processes to determine process improvement plans and quality assurance
Provides oversight, accountability, and support to achieve expected performance and productivity.
Communicates effectively and frequently with University students regarding initiatives, events, successes, needs for improvement, process changes, and changes to general expectations.
Develops process improvement plans, corrective action plans, and mitigation plans in response to missed operational and scholastic targets.
Ensures standardized execution of processes including performance audits, quality reviews, supervisory home visits, initial introduction home visits and compliance adherence
Demonstrates regard and maintains confidentiality of all information for Community Care processes in accordance with AHS policy and HIPAA regulatory requirements.
Supports the production of accurate population-based reports for defined client populations
Participates in committees, teams or other work projects/duties as assigned.
What You Will Need:
KNOWLEDGE AND SKILLS REQUIRED:
Comprehensive knowledge and understanding of student mentorship, colligate level teaching and course development
Comprehensive knowledge and understanding of case management, care coordination, and managed care
Ability to work effectively with physicians and other health care professionals
Excellent organizational skills
Excellent written and verbal English communication skills
Ability to work with people of various backgrounds and maintain good interpersonal relationships with a large department staff, medical staff, administration, University staff, providers, employers, and clients.
Intermediate or higher level of experience with Microsoft Word, Excel, Outlook, PowerPoint, and SharePoint
At least 1 year of experience as a telecommuter or working in a home health/hospice environment preferred.
At least one year of facilitating webcam enabled meeting using Skype, WebEx, Zoom or similar technologies preferred.
EDUCATION AND EXPERIENCE REQUIRED:
Master of Science in Nursing, Master of Social Work, Master of Mental Health Counseling, Master of Public Health or other master’s degree in a health care related field
3-5 years of college level teaching experience
3 years of student mentorship or 4 years of combined college level teaching and mentoring experience
5 years of Population Health, Care Coordination, Transitions of Care, and/or Case Management experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Nursing Licensure as a professional registered nurse (RN) in good standing with the State of Florida
OR Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC)
OR Licensed in other health care related field as applicable
Certified Case Manager (CCM) preferred.
Under the direction of the Community Care leadership team, the University Liaison is a divisional role that provides supervision and leadership to all University students, including class instruction, recruitment, onboarding, orientation, education, and performance management of assigned students. The University Liaison will support the organizational mission and operational goals of the Community Care Team by routinely analyzing and interpreting all available data and reporting, maintaining productivity expectations, while routinely monitoring/auditing University students for production of high-quality, person-centered, collaborative work. Evaluates current processes to determine appropriate recommendations and updates. Responsible for the direction and oversight of University students throughout the Central Florida Division requiring travel to multiple AdventHealth locations for class instruction, the supervision of University students in both the office and community home visits. Responsible for ensuring quality interventions that result in the delivery of best-in-class case management and care coordination outcomes including maintaining compliance with regulatory requirements. Attends pertinent seminars, workshops and professional meetings to maintain relevant knowledge regarding population health management and leadership trends. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
