VA Manchester Healthcare System in Manchester, NH is looking for 4 Registered Nurse Case Managers for a TIME LIMITED, FULL-TIME APPOINTMENT, NOT TO EXCEED SEPTEMBER 30, 2022 in the Office of Community Care Service Line. The responsibilities of the Registered Nurse for Case Management include, but are not limited to the following: Provides initial and ongoing assessment of patients to identify needs, care goals, and appropriate resources necessary for case management. Demonstrates knowledge in using VA Medical Center and community resources to provide the highest quality of care in the most cost-efficient settings. Provides leadership in the application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance/improvement. Possesses thorough knowledge of utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure positive outcomes and delivering quality patient care. Demonstrates knowledge and ability to apply principles of evidence-based practice. Provides support at the unit level, the service level, and the organizational level. Collaborates with the Patient Aligned Care Team and Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient).
