Supervisory social workers typically have significant personnel management responsibilities for a small to moderate sized professional and non-professional staff at the GS-12 grade level and below. The supervisory social worker is responsible for the professional and administrative management of an area in a social work service, in a care/product line, or multiple facilities. They provide direct clinical services with or without program coordination responsibilities. The major duties of the position includes, but are not limited to: Providing oversight of GPD-funded program participants' care and, at times, case management of those participants at sites where organizations are receiving per diem payments; which includes monitoring the care in, and assessing the compliance of, the program receiving per diem as outlined in the recipient's original grant applications; intervening to facilitate compliance, or correction, of the program, when appropriate; and performing a cursory environmental review to identify hazards or other deficiencies to be corrected. Verifying Veteran-status and eligibility of program participants. Assisting in assessing the needs of Veterans assigned and document the social support services provided utilizing patient centered care approach to assure Veteran is involved in decisions regarding their healthcare. Participating in collaborative treatment planning with the Veteran, family and the multi/interdisciplinary treatment team; prior to team meetings, reassess the Veterans current psychosocial situation, to include financial, residential status, coordinate any equipment needs and other needs that may affect discharge. Providing high-risk psychosocial assessment in crisis intervention, seeking to address the cause as well as the presenting complaint; communicating with the consulting team, Veteran/family psychosocial resources available. Assessing for and providing care or case management appropriate to the assessed level of care. Utilizing Event Capture code approved for social workers for inpatient care and uses Appointment Management/Encounters for outpatient clinics following the VA policies and processes for appointment scheduling. Develop a post-discharge plan with Veteran to improve outcome of transitioning into permanent housing and reduce likely of repeated episodes of homelessness. Provides the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluations; selection of staff, training of staff, and recommendation of awards, advancements, and when appropriate, disciplinary action.
