We are currently seeking a Full Time Referral Service Manager. This person provides accountability and responsibility for excellence in the operational office setting and clinical practice of care transitions in an ANCC Magnet(r) designated Christiana Care Health Services area. The Referral Service Manager is responsible for managing patient referrals from hospitals and other referral sources to ensure a smooth transition of care for patients being admitted to Christiana Care HomeHealth. The successful candidate will work the hours of 8am - 4:30pm. Will require an occasional weekend and holiday shifts, as well as some on call shifts.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Demonstrate knowledge and skills necessary to provide care to adolescent, adult and geriatric patients
Develops and maintains methods and processes to ensure exceptional patient satisfaction and customer experience
Routine supervision of approximately 20 HomeHealth clinical and non-licensed professionals
Providing clinical record documentation review, case conference and patient rounding to establish visibility, teamwork and monitoring performance
Supports and communicates system-wide and agency based Performance Improvement initiatives
Promotes the autonomy of the clinical staff, encouraging independent decision making and problem solving
Responsible for service recovery and analyzing gaps in quality of care transitions
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's Degree in Nursing required; Master's Degree preferred and must agree to complete within 3 years of start
RN licensure in State of DE, or an appropriate compact license
3 years of management experience or demonstrated progressive leadership development
2-3 years of Home Health/Hospice, or Case Management experience
