The RN Case Manager provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being. Documents initial and ongoing patient assessment. Uses the nursing process to formulate safe and effective care based on each individual's unique needs (including psychosocial, spiritual, cultural, bio-physical, educational, and age specific needs). Exhibits responsibility and accountability for care of a group of patients. Provides comprehensive and coordinated nursing care to Urology patients. Collaborates with services internal and external to the VA system of care to effectively meet the health promotion/disease management, acute, chronic and long-term needs, based on the Veteran 's goals and plan of care. Coordinates, delegates, and directs both professional and non-professional Personnel. Prioritizes care based on acuity level, patient/family needs, available resources of time, personnel, supplies. and equipment. Documents care appropriately. Effectively communicates with patient and family members in crisis situations. Involves patient/family in care decisions and provides education based on identified needs. Performs effective working relationships with all health care team members, patient and family members. Protects and respects patients' rights and confidentiality.
