The Primary Care Mental Health Integration (PCMHI) Nurse practices in the Outpatient Mental Health Clinic as a Mental Health Specialist assigned to the PCMHI team which is co-located with the Primary Care outpatient clinic providing psychiatric care for Veterans, including substance abuse, dementia, major mental illnesses, and personality disorders. Care may be delivered in person, in groups, by phone, or by video. The duties and responsibilities of the PCMHI Nurse includes but are not limited to the following: Triaging new patient referrals, case management, and consultation. Facilitating follow-up care for established patients. Collaborating with treatment providers and managing the medical center's mental health consults. Co-managing and providing evidenced based mental health nursing care to Veterans who have significant mental health issues who are followed by Mental Health and Neurosciences Division and Primary Care. Providing nursing clinical leadership and consultation with tools to improve and sustain the quality and effectiveness of care for depression, anxiety, PTSD, smoking cessation, substance abuse and other mental health disorders in conjunction with PCMI and Primary Care providers. Experience in mental health nursing as well as knowledgeable of facility and community resources. Work Schedule: Monday - Friday, 8:00 am to 4:30 pm Financial Disclosure Report: Not required
