The Senior Social Worker located at Saginaw VAMC independently provides clinical psychosocial and case management services at an advanced practice level to eligible Veterans and their family members to meet biological, psychological, social and environmental needs. Typically, the incumbent practices independently in an ambulatory care setting with minimal clinical supervision. Supervision provided by a social work supervisor is administrative and professional in nature. The Senior Social Work possesses the knowledge and skills to determine priority for services. The Veterans served may present with complicated health care and psychosocial needs that require a high degree of clinical oversight and creative problem solving. The Senior Social Worker is responsible for providing community based mental health treatment and psychosocial intervention to address a broad range of medical and mental health issues. The incumbent serves Veterans who tend to have serious, frequent and severe crises, lack family or adequate community support networks, frequently fails to comply with instructions and treatments; have significant deficits in coping skills and require continuing psychosocial support. The Senior Social Worker provides a continuum of care encompassing crisis intervention, advocacy, care coordination and collaboration, and evidence-based care to a specialized patient population. The incumbent must collaborate with community partners and maintain current knowledge and information regarding mental health and social work services in the community. The Senior Social Workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advance practice level. At least two years should be in an area of specialized social work practice and demonstrate a range of skills including providing. Duties include but are not limited to: The Senior Social Worker performs specialized treatment of complex physical or mental illness. They are responsible for the overall function of program development, recruitment of Veteran patients, case management, networking with community resources, and coordination of care with the mental health and medical staff on site as well as various departments and community facilities in the primary service area. The Senior Social Worker provides leadership, direction, orientation, coaching, in-service training, staff development and continuing education programs. They serve as the subject matter expert on the psychosocial treatment of pain. The Senior Social Worker participates in gathering relevant data, and identifying areas for program improvement. They serve on committees, work groups, and task forces at the facility, VISN, national level, or in the community as deemed appropriate by the supervisor or the Chief of Social Work. These complex advance practice duties are performed by the Senior Social Worker a minimum of 25% of the time. The incumbent coordinates screening of Veterans requesting services related to MST. The Senior Social Worker has extensive knowledge of medical center policies and procedures, VHA and VBA regulations as applicable to the program they serve. The Senior Social Worker receives and completes consults from interdisciplinary teams and other social workers on complex difficult cases using advance practice skills and expertise. Incumbent is able to provide, crisis, individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with serious disorders. Incumbent also provides consultation to referral sources and is responsible for development and maintenance of professional standards of practice and educating others regarding same. The Senior Social Worker documents independently in CPRS electronic medical records, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referrals. The Senior Social Worker interviews and conducts psychosocial assessments to establish Veteran's treatment needs, presenting problems, and DSM V diagnoses. The incumbent assesses high risk factors, acuity and need for services. The Social Worker interprets and explains VA treatment and benefit programs. It is the responsibility of the incumbent to develop a treatment plan with the Veteran based on this assessment. The incumbent along with the Veteran will develop the treatment plan that includes measurable time limited treatment goals. The treatment plan will be reviewed during therapeutic sessions and reassessed as appropriate. Work schedule: 8:00 AM to 4:30 PM (Monday through Friday) Compressed/Flexible Schedule: Not Available.
