The Gulf Coast Veterans Health Care system (GCVHCS) is seeking a Registered Nurse (RN) Navigator to serve as a coordinator in a multidisciplinary environment interacting directly with Ambulatory Care Service, Medicine Service, Nursing Service, Surgery Service and contracted or affiliated hospitals which provide care for VA patients. The RN Navigator will be part of the Office of Community Care. The RN Navigator is responsible for planning, coordinating, and performing a wide range of case management, clinical, administrative, program, and technical functions to support quality of care for Veterans. Duties include but not limited to the following: Demonstrates leadership and initiative in delivering and improving holistic care through collaborative strategies with others, ensuring compliance with the mission, vision and values of the medical center and/or (Veterans Integrated Service Network (VISN). Possesses the knowledge and skills to effectively apply all aspects of the nursing process with collaborative, interdisciplinary practice, which will then enable effective care coordination and implementation of health promotion and disease. Provides support to Primary Care by ensuring that coordination of care is performed in a timely manner and by collecting, retrieving, and analyzing data as appropriate. This position requires flexibility and dependability. Also, requires adaptability with role and incumbent may perform other duties and responsibilities when there is a need. Works directly with the patients, VA staff, non-VA care providers and organizations.
