The CBOC Supervisory Social Worker will serve in combined supervisory and clinical roles at the assigned Community Based Outpatient Clinics aligned with the Dorn VA Medical Center in Columbia, SC. The Patient Aligned Care Team (PACT) Social Work Supervisor will provide high quality medical social work care and services to assigned Veteran patients enrolled in outpatient Primary Care. The incumbent provides active case management and care coordination. clinical assessment and treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers/agencies as needed by the client. This is accomplished in cooperation with other members of the Primary Care/ PACT treatment team and/or other interdisciplinary teams as appropriate. The incumbent is expected to function above the Full Performance Level and demonstrate the capacity to work independently in the clinical setting. The CBOC PACT Social Work Supervisor will also serve as a Supervisory Social Worker within the Social Work Service at the Dorn VAMC with supervisory authority and responsibility for GS-9and GS-11 Social Workers working within the CBOC outpatient PACT clinic team as assigned. This supervisory position will be geographically located in Columbia, SC. This incumbent will travel between the main facility in Columbia and the three CBOCs: Florence. Sumter, and Orangeburg on a daily basis. This supervisor will travel to each lower state CBOC on alternating days weekly. The incumbent will be responsible to provide administrative and clinical services at a different lower state CBOC each day, each week, including 1 day at our main facility in Columbia and 2 days at one of the lower state CBOCs and the other 2 days split between the remaining 2 CBOCs. This position requires 75% or more travel. Work Schedule: Monday - Friday, 8:00 am to 4:30 pm. Subject to change based on the needs of the agency. Financial Disclosure Report: Not required
