Columbia is the geographic center of South Carolina, with easy access to mountains (two hours west) and nationally recognized beach resorts (two hours east). Columbia boasts a nationally acclaimed library system, highly rated schools, and many outstanding golf courses. Big time college sports abound with two major schools - the University of South Carolina, Columbia, in the Southeastern Conference and Clemson University, Clemson, Atlantic Coast Conference (two hours away). The initial cut-off date for referral of eligible applications will be 1 February 2021, with subsequent cut-off dates every Monday. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. The RN Mental Health Intensive Case Manager demonstrates clinical expertise, leadership, and creative approaches to management of complex care for seriously ill mental health clients within a 50 miles radius of the Medical Center or 65 miles from CBOC Range. In addition, applies clinical skills of case management which emphasizes a collaborative process that assesses, advocates, plans, implements, coordinates, monitors and evaluates health care options and services so that they meet the needs of the individual patient. Intensive acute case management is geared towards Veterans with the highest level of need. These Veterans may be hospitalized or have had recent, extended or repeat hospitalizations or recurrent episodes of crisis.
