Applicants will primarily work at the Overton Brooks BA Medical Center in Shreveport, Louisiana providing Clinical Social Work Interventions via Video Connect (VVC) visits to Veterans receiving services at the Texarkana CBOC. This position will require travel to the Texarkana CBOC to conduct face to face interventions with Veterans 1 to 2 time per week as needed. Announcement extended for additional application input. - The Senior Social Worker/PACT Social Worker (SW) Case Manager (CM) has practice skills and judgment, demonstrating progressively more professional competency. Services include, psychoeducation, crisis intervention, advocacy, high quality case management services, outreach activities, and coordination of linkages to other appropriate VA or community service providers / agencies as needed. - The Senior Social Worker/PACT Social Worker (SW Case Manager (CM) independently provides expert consultation and guidance to colleagues, expands clinical knowledge in the profession, role models effective social work practice skills, teaches or provides orientation to less experienced social workers, develops innovations in practice interventions, and provides clinical supervision for less experienced social workers who are pursuing independent licensure or certification. - Conducts a full bio-psychosocial/ assessment of the Veteran with specific examination of current coping skills, emotion regulation, and support networks in place. Prior history of mental treatment is also considered, in addition to the assessment of complexity of treatment (number of traumas, current life stressors, severity of mental health symptoms). Previous and current substance abuse is evaluated in addition to past and current suicidalideation. - The PACT Social Worker (SW) Case Manager (CM) /Senior Social Worker also conducts timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. - Assists Veterans, their families and caregiver in resolving psychosocial, emotional and economic barriers to health and well-being while building on their strength and their abilities. The PACT social worker also provides interventions independently with Veterans and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems. - Utilizes outcome evaluations to further treatment and designs system changes based on empirical findings and in consultation with other specialty team members. - Provides consultation to students and colleagues regarding training opportunities and procedures. - Will use VA Video Connect (VVC) to increase access to care for Veterans. Will act as a consultant to other PACT and CVT team/staff members, other clinicians within the medical center, within the VISN. - Is an active participant in the treatment planning process with other disciplines. - Will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. - Will provide individual, family and group counseling as appropriate. - Will provide consultation/education to medical residents on social work practice and VA policy and procedure. - Will act as liaison between VA and community resources, to include marketing and public relation duties as needed. - Will provide case management to Veterans and families, as needed, throughout the continuum of care. - Will address Advance Directives and Organ Donation in accordance with the standards of their work area. - Will provide wellness/prevention education and facilitate Patient/family support groups as appropriate. - Will coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. Educational Duties - Will as appropriate, supervise the clinical practice of social work students, or a less experienced social worker. - Expand clinical knowledge in the profession, provide consultation and guidance to colleagues. - Be a role model of effective social work practice skills to less experienced social workers. - Teach and provide orientation to less experienced social workers. - Develop innovations in practice interventions and provide clinical supervision for social work licensure or certification. - Maintains current certification requirements through either an advanced generalist board or clinical Association of Social Work Boards (ASWB) examination. 3. SUPERVISORY CONTROLS The GS-12 Senior Social Worker reports to the PACT Social Work Supervisor, but practices independently, exercising initiative and Independent judgment in day-to-day activities. Supervision and consultation are of an administrative and professional nature and includes the PACT Social Work Supervisor, the Assistant Chief of Social Work Service (SWS), and the Chief of Social Work Service (SWS), with the Chief and Assist Chief, SWS providing guidance/mentoring related to social work practice from the Social Work Professional Practice Office, as part of the larger Health Care System. Work Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m. (Subject to change based upon agency needs.) Financial Disclosure Report: Not required
