This position is assigned to the Social Work Service at the Madison VA in the Mental Health Outpatient Clinic. The incumbent will be part of an interdisciplinary mental health treatment team. The Social Worker in this position functions within the Mental Health Clinic (MHC). Additional Duties: Ability to conduct psychosocial assessments and provide psychosocial interventions/treatments with supervision to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Ability to accurately assess psychiatric conditions according to the DSM-V. Accurately assesses veterans who present with psychiatric and psychosocial emergencies. Incumbent provides crisis intervention services, including timely and appropriate interventions to prevent harm to self or others, as well as coordination of family involvement when indicated. Identifies high-risk patients and provides case management services. Establishes and maintains effective therapeutic relationships with veterans and their families. Independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. General knowledge of veterans' benefits and services relating to special programs, service-connected compensation, non-service-connected pension. Plans treatment interventions and enters treatment plan in the medical record within established time frames. Work Schedule: Monday - Friday, 8:00am - 4:30pm Telework: May be available.
