JOB SUMMARY: Under the immediate supervision of the Clinical Director. Functions as the Referral and Intake Coordinator with responsibilities for the day to day oversight of the Referral and Intake team. Oversees quality of assessment and functioning, gathers referral and intake data, compiles reports and provides data to the Director. Responsible for providing assessments and coordinating admissions, completing pre-authorization and paperwork related to the admission. May provide back-up by answering the phone and taking initial calls. May coordinate with outreach liaison and provide marketing and development contacts in the community. Will assist nursing staff with clinical duties as needed.

CASE MANAGEMENT (INTERMEDIATE): - Demonstrates mastery of case management in practical applications for complex clinical care coordination. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.
CONSULTING SERVICES (NOVICE): - Possesses sufficient fundamental proficiency to successfully demonstrate consulting services including appropriate knowledge of core technology, products, services and operations within field of work, in practical applications of moderate difficulty. Is aware of industry trends. Attends industry seminars and forums. Understands the connection between assignments and the missions and goals of the department.
CLINICAL PATIENT DOCUMENTATION (INTERMEDIATE): - Role models patient documentation in practical applications of a complex nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering patient documentation services without requiring support and instruction from others. Able to train and educate peers by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job. Contributes to the nursing assessment by collecting, reporting, and recording objective and subjective data in an accurate and timely manner.
