Nurse Navigator – Mayo Practice Associates (Full Time)
We are currently seeking a Nurse Navigator for our Mayo Practice Associates. This 40 hour a week position is responsible for the delivery of patient care and is accountable for assessing, planning, implementing, and evaluating needs of the patient. Assumes accountability for individual patient outcomes by providing direct care and supervising care delegated to team members. The nurse navigator will provide coordination of staff in the practice setting and has the authority to assign employees and the responsibility to direct employees in the workplace. Must exercise independent judgment while performing job functions on a regular basis, and this is a substantial part of the overall job responsibilities. Must be a graduate of an accredited school of nursing with current licensure in the State of Maine, and BLS certified. Must be ACLS certified or acquire within 6 months of hire. Three years clinical experience as RN or LPN. Must have excellent written and verbal communication skills and able to function as a positive and effective team member in a dynamic environment. Basic computer skills required.
