Responsible for monitoring the delivery of care to patients identified by case finding criteria and/or referral for case management services.Identifies variances and assists in the development of strategies to reduce variance patterns.Ensures appropriate utilization of facilities and services and provides for the review of admission, continued stay and professional services rendered.Coordinates discharge plan and needed social service and/or chaplain services.Ensures planning to facilitate patientï¿½s return to their optimum level of function.Performs data collection as indicated for Commitment to Quality.Performs other duties as required.
Required: Associate of Science in Nursing (ADN).
Preferred: Bachelor of Science in Nursing (BSN).
Required: Five (5) years acute hospital patient care experience.
Required: Active Registered Nurse license and current Basic Life Support. Preferred: Accredited Case Manager (ACM).
