Works in collaboration with patients' primary care physician team/office, hospital care managers, Care Transitions Team, ambulatory care managers, and other care partners to provide a model of patient care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Complete health risk assessments; perform medication reconciliation; assess social determinants of health and coordinate services as needed. Provide Case Management support for assigned caseload of Medicare ACO beneficiaries and HMO members.
A.A. Diploma, B.S.N preferred from college or university and one or two years' related experience and/or training. Current California RN license required.
