ElderServe Health, Inc. is looking for a full time Nurse Care Manager to join our dynamic team in our Bronx, Brooklyn and Queens offices.
Nurse Care Manager will be responsible for the care management of a defined population with the plan; within that, responsible for care planning that is of good quality and appropriate to the needs of the members, for determining that members receive the services they need, and that services are acceptable and accessible to the member; assures that the care plan changes as members’ needs change. Evaluate clinical assessments in developing the care plan.
Must have a New York State RN license and registration, Bachelor of Science degree and have at least 1 year of experience in a MLTC or PACE program. Valid driver’s license and vehicle required
