Under direct supervision, the Therapist provides services in the adult intensive outpatient level of care that includes psychological assessment, evaluations, diagnostic, therapeutic, and/or case management services for individuals with Dual Diagnoses. The majority of services will be provided through group interactions, but may also include individual therapy, family therapy, and family groups.
Develop, implement, and administer therapeutic treatment plan by gathering information to complete integrated assessments for medical and psychological history and compile all pertinent documents to facilitate treatment plan.
Participate in weekly treatment team sessions.
Observe patient for abnormal physical or mental behavior and monitor progress with components of treatment plan.
Conduct and document group therapy sessions according to treatment plans and individual therapy sessions as needed.
Contact and schedule counseling sessions with family, significant others, and involved agencies.
Act as liaison with family and other agencies to help patient maintain success in the community.
Perform daily tasks of counseling by instructing patients in progress charting.
Respond to any emergency situation involving patients in treatment.
Participate as a team member by cooperating with staff and demonstrate compromise and expression of opinions as appropriate.
Attend staff meetings as required and perform duties as a positive role model for patients and colleagues.
Utilize chain of command in all aspects of program and follows organizations policies and procedures.
Perform community relations by ensuring all written and verbal information is clear, concise, legible, and accurate.
Communicate and cooperates with families, relevant external personnel, and staff members.
Document all external and internal contacts and relays patient information according to company and governmental guidelines maintaining appropriate confidentiality.
Primarily provides evidence-based treatment groups for people receiving intensive outpatient services.
Provides counseling, education, therapy and/or psychotherapy to consumers and families as appropriate to the position.
Prepares treatment plans, discharge plans, and follow-up care programs. Provides therapeutic crisis intervention and emergency services as required.
Maintains thorough and accurate documentation of clients' progress and response to treatment.
Provides care to consumers in a manner that promotes safety, comfort, and the therapeutic environment.
Maintains confidentiality of consumer records and documents intervention and program activity according to company policy.
Attends and participates in group sessions, in-service education and training opportunities, meetings, supervisory sessions and special events.
Provides case management services as necessary.
Participates in the review of clinical issues and program policies and procedures
