: A Licensed Masters Social Worker or Licensed Professional Counselor with strong leadership abilities, communication, and problem solving skills who, with minimum supervision, collaborates with all members of the treatment team in assessment of patient needs related to discharge planning and coordinates interventions with other members of the treatment team to meet those needs in a quality cost effective manner. Completes psychosocial assessments, family therapy, individual therapy, treatment planning, with the ability to apply diagnostic criteria as outlined in the DSM-5. Collaborates with Utilization Review (RN) to ensure requirements for funding sources are met as well as to ensure that legal process requirements are met for patients on involuntary status Supports an organizational culture for Service Excellence and practices the Service Excellence and practices the Service Excellence Standards to all customer groups
