Transplant Coordinator III/NP
This position is responsible for the management of patients needing dialysis access or having problems with their dialysis access as well as other surgical patients, including kidney transplant, in collaboration with the transplant surgeons. The TC III/NP must possess decision making skills regarding dialysis access management and transplant as it related to patient education. Specifically, he or she will have direct experience in working with the chronic kidney disease (CKD) population; direct experience with dialysis and/or transplant patients and direct experience developing and implementing orders in collaboration with members of the multidisciplinary team. This position requires the TCIII/NP to provide pre-op and post-op education related to dialysis access, kidney transplant, surgery, post-op care, and significance of laboratory tests and procedures. This position requires strong communication and team building skills and an ability to participate in continuous quality improvement and education. The TC III/NP is responsible for transplant clinic oversight with the post-transplant coordinators. The employee shall participate in required preventive medical and medical monitoring programs.
Graduation from an accredited school of nursing with Master's degree.
Graduation from an accredited nurse practitioner program.
National Nurse Practitioner Board Certification.
Possession of a valid California license as a registered nurse.
Possession of a valid California Nurse Practitioner license.
Possession of a valid DEA number and furnishing certificate.
Possession of valid NPI number.
Current CPR or Basic Life Support Certification.
Case management, transplant or nephrology experience required.
CITI certification within 3 months of hire if working with human subjects in research studies.
Advanced Cardiac Life Support Certification.
Certified Nephrology Nurse or Nurse Practitioner Certification (CNN or CNN-NP).
Certified Clinical Transplant Coordinator or Nurse (CCTC or CCTN).
On-call duties, working weekends, holidays or overtime may be required.
