The Case Manager coordinates patient-focused care in a cost-effective and quality manner. The Case Manager offers alternative means of care to patients who are determined to be in need either because they are about to be discharged or require long-term care. Advocates on behalf of the patient while working directly with administrators and physicians, to offer high-quality resources to the patient.
Essential Duties and Responsibilities:
Facilitates progress through the continuum of care, from pre-admission through discharge
Evaluates patients for discharge planning needs, and appropriately directs the patient’s care. Functions as a patient advocate to assist patients in resolving concerns
Provides feedback to payers on patient’s clinical status, and secures approvals for additional days of stay
Collaborates with patient care team and outside facilities to help facilitate appropriate discharge planning
Promote quality and cost-effective interventions and outcomes
Monitors resource consumption for most effective use
Identifies teaching and psychosocial concerns for the best patient outcomes
Assists in aggregating data for quality trending
Adherence to Best Practices including CMS, Joint Commission and State laws governing medical necessity and adherence to the Case Management Standards
Leads UR Committee. Must work within scope of practice.
