Glen OaksÂ Hospital is seeking a dynamic and talentedÂ Outpatient Therapist. The Therapist will provide clinically indicated psychotherapy to individuals and groups. While providing the best customer service experience, the Therapist provides customer service to patients, families, allied health professionals, physicians and staff from all hospital departments.
Key Responsibility Include:
To maintain documentation of therapeutic services provided.
To provide case management to assigned patients.
To recognize signs and symptoms of chemical dependency disorders and apply/utilize appropriate treatment Iinterventions.
To recognize signs and symptoms of mental health disorders and apply/utilize appropriate treatment interventions.
To keep psychiatrists and treatment team informed of patientsâ€™ status.
To participate in and to document clinical supervision, if applicable.
To attend team and therapy meetings; to provide consultation to treatment team.
To assess all chemical dependency needs of the patient, using the twelve (12) core functions.
To develop treatment plans.
To provide clinical programming for patients and families.
To complete psychosocial history.
To interface with Utilization Review on a regular basis to determine patient treatment needs within managed care counseling.
To complete discharge planning requirements.
