Nurse Care Manager I – Care Management
We are seeking an experienced RN who is passionate about applying their nursing expertise and outstanding communication skills to enhance the care of patients and their families. Collaborate with physicians and the health care team to manage a complex caseload
Promote the effective utilization of services and care coordination for all patients
Assess patients’ health care needs, review plans of care, identify barriers to the effective utilization of resources, and evaluate patients’ needs throughout the continuum of care
Coordinate social services to address patients’ psychosocial needs
Initiate and promote efforts to enhance patient flow and decrease length of stay
Provide education for staff
Maintain a quality-based, person-centered approach to achieve departmental and institutional goals and process improvements
Current State of Illinois Registered Nurse licensure
Bachelor of Science in Nursing required; master’s degree in Nursing or related health discipline preferred
Three or more years of experience as a Registered Nurse, Nurse Practitioner or Nurse Case Manager
Able to act as a team resource for clinically complex cases
Excellent interpersonal and collaboration skills
Case Management certification preferred, or able to commit to obtain within 2.5 years of hire
Current CPR certification preferred
