PCG is currently seeking a Case Management Supervisor to be located in our Albany, NY office to help launch an exciting new healthcare administrator project. The Case Management Supervisor will have responsibility overseeing a team of 5 to 7 full time case managers that will be charged with helping program recipients, family members, and caretakers navigate healthcare benefits offered through a publicly administered Medical Fund on behalf of a state agency. The Case Management Supervisor will have the opportunity to shape the program from its inception as Public Consulting Group, Inc. assumes operations from a prior vendor. The purpose of the Fund is to provide a funding source for future health care costs associated with birth?related neurological injuries.
The Case Management Supervisors will develop program policies and procedures, help in the recruiting of staff, finalize system needs, and ultimately be given the opportunity to make a lasting impact on program recipients by identifying and making improvements on how the program is managed and administered. The Case Management Supervisor will oversee the team to perform the Fund operations, including, performing assessments, providing case management services, assisting families and program recipients through prior approval of certain healthcare services, and collaborating with our claims administrator team. Successful candidates will be accountable for the full patient care cycle. The Case Management Supervisors will oversee the duties of Case Managers including assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a requirement.
In addition, PCG is looking to expressively expand our capabilities in this service offering and the Case Management Supervisors will be afforded the opportunity to help scale operations and participate in program expansion strategies.
Lead, supervise, and mentor a team of Case Managers.
Participate and lead client meetings, getting exposure to State program and healthcare policy decision makers, during the implementation of program operations, as well as throughout the continuing operation of the program.
Develop policies and procedures to effectively coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered to program recipients
Assist in the development of service plans specific to the Fund program requirements
Help vet the purchasing of Case Management software to leverage to perform functions.
Oversee and facilitate multiple care aspects (case coordination, information sharing, etc.)
Develop processes that help Case Managers empower patients to make informed decisions by acting as their advocate regarding their clinical status and treatment options
Develop effective working relations and cooperate with consulting staff, project managers, and the medical team throughout the entire case management process
Take the extra mile and to ensure our firm’s interaction with patients exceeds expectations and achieves a level of excellence
Ensure proper and accurate record documentation is maintained by Case Managers
Promote a culture of quality and cost-effective interventions and outcomes
Assess and address motivational and psychosocial issues
Adhere to professional standards as outlined by protocols, rules and regulations
BS degree in related healthcare field
Current license, certification or registration as a Registered Nurse or Social Worker
Certification in Case Management is required
1-2 years pediatric experience required, pediatric hospital or home health experience preferred.
Excellent communication, interpersonal and client-facing skills.
Ability to work independently and on a team
Proven experience in case management, including as a nurse, medical, mental health care manager or a related job
Experience or educational training in health or social services and the development of case management plans
Excellent knowledge of case management principles, healthcare management and reimbursement
Previous experience with psychological aspects of care
Effective communication skills
Excellent organizational and time management skills
Familiarity with professional and technical emerging knowledge
Problem solving skills and ability to multi-task
